Searching for the best olive oil for health returns a wall of milligram figures — 500, 650, 742 — each presented as proof that one bottle is healthier than the next. The numbers are not meaningless, but they are far less informative than they look, because almost none of them come with the two pieces of context that would make them usable: how they were measured, and when. This article sets out the one health claim on olive oil that carries a legal threshold, what the diet research actually established, and the handful of markers genuinely worth checking before you buy.

The Short Answer
- The healthiest olive oil is a fresh, genuinely extra virgin, minimally processed oil that you actually use daily — not necessarily the one advertising the largest polyphenol figure.
- Only one olive oil health claim carries a legal threshold in the EU and UK. Commission Regulation (EU) No 432/2012 permits the claim that olive oil polyphenols contribute to the protection of blood lipids from oxidative stress, and only for oil containing at least 5 mg of hydroxytyrosol and its derivatives per 20 g of oil.
- A polyphenol number without an analytical method and a measurement date tells you very little. Different labs use different methods and report different units, and phenolic content falls steadily from the moment the oil is pressed.
- The strongest evidence for olive oil concerns the Mediterranean dietary pattern, in which extra virgin olive oil is the principal fat — not any single brand or bottle.
- Grade, harvest date, extraction method, packaging and storage do more to determine what a healthy olive oil delivers in your kitchen than the country of origin does.

What "Healthiest" Actually Means
The healthiest olive oil is a fresh extra virgin oil, mechanically extracted and stored away from light and heat, used regularly in place of less favourable fats. Grade and freshness set the ceiling; polyphenol content is what fills it. Everything else on a label — origin, awards, imagery — is secondary to those two things.
That is a deliberately unglamorous answer, and it is worth understanding why the alternative answers are weaker. Regulators in the EU and UK have approved exactly one health claim tied to olive oil polyphenols, and it is narrow and quantified rather than sweeping. Under Commission Regulation (EU) No 432/2012, a producer may state that olive oil polyphenols contribute to the protection of blood lipids from oxidative stress, but only where the oil contains at least 5 mg of hydroxytyrosol and its derivatives — the oleuropein complex and tyrosol — per 20 g of oil. The regulation also requires the consumer to be told that the beneficial effect is obtained with a daily intake of 20 g of olive oil.
That threshold is the closest thing the category has to an objective bar. Notice what it does not say: nothing about disease, nothing about brands, and nothing that improves as the number climbs from 500 mg/kg to 750 mg/kg. The claim is binary — either an oil clears the threshold or it does not.

Polyphenols: Why the Number Is Only Half the Story
Polyphenols are the compounds behind the bitterness and the peppery catch in the throat, and they are the reason anyone talks about olive oil and health at all. Oleocanthal, oleuropein, hydroxytyrosol and tyrosol are the ones most often named. Three things undermine a bare figure. First, method: total-phenol assays and the targeted hydroxytyrosol-and-derivatives analysis required by the EU claim are not the same measurement, and they do not produce comparable numbers. Second, timing: phenolics degrade continuously after pressing, so a figure measured at the mill in November describes an oil that no longer exists by the following summer. Third, sampling: one batch tested once is not the same as per-batch testing.

None of this means high-polyphenol producers are being dishonest. Many are doing something genuinely difficult — early harvesting, fast pressing, careful storage — and the numbers reflect real work. The point is narrower: a number you cannot interrogate is a marketing asset, not evidence.

What the Mediterranean Diet Research Does and Does Not Show
The single most cited piece of evidence in this category is the PREDIMED trial, and it deserves to be described accurately rather than waved at. Estruch and colleagues published Primary Prevention of Cardiovascular Disease with a Mediterranean Diet Supplemented with Extra-Virgin Olive Oil or Nuts in the New England Journal of Medicine in 2018. It is a republication: the original 2013 paper was retracted after a review found irregularities in how participants had been randomised, including a site that randomised whole clinics rather than individuals and several hundred participants assigned non-randomly alongside household members. The authors re-analysed the data and the republished paper reported broadly similar findings, with the Mediterranean-diet groups showing a lower incidence of major cardiovascular events than the control group.
Two honest conclusions follow. The evidence base for the Mediterranean pattern remains the strongest of any traditional dietary pattern, and it was tested with extra virgin olive oil as the principal fat. But it tested a whole eating pattern, not a bottle, and its flagship trial needed correcting — which is exactly why nobody should be selling you a specific oil on the strength of it. The Five Markers of a Healthy Olive Oil
Strip out the marketing and a healthy olive oil is identifiable from five things, none of which requires you to trust a brand's story.

Grade is the one most often misread. "Pure olive oil", "light olive oil" and "classic olive oil" are all refined products despite the reassuring names, and refining removes the great majority of the phenolic content. Storage and Use Change the Answer
An oil that clears every marker above will still lose most of its advantage in a kitchen that mistreats it. Light, heat and oxygen are the three enemies, and a bottle kept on a sunny windowsill next to the hob is being degraded continuously. Store it in a closed cupboard away from the cooker, keep the cap on, and buy a size you will finish within a few months rather than a large tin that sits open for a year.
Heat in cooking is more nuanced than the internet suggests. Extra virgin olive oil is more stable under normal cooking temperatures than its reputation implies, largely because its phenolic and monounsaturated profile resists oxidation — but sustained high heat does reduce phenolic content, so the sensible pattern is to cook with it freely and also use some of it raw, over salads, bread or finished dishes, where the compounds arrive intact. Two objections are worth answering directly. "Isn't a polyphenol number better than nothing?" Yes, marginally — a producer who measures at all is usually a producer who cares. But treat it as evidence of intent rather than as a comparison metric, unless the method and date are given. "If the research is about a whole diet, does the specific oil matter at all?" It does, but downstream of the pattern: choosing extra virgin over refined, and fresh over old, changes what you are actually consuming, whereas moving from a 500 mg/kg oil to a 700 mg/kg oil is a much smaller change than eating more vegetables and less processed food.

A modest extra virgin oil pressed last autumn and kept in a cupboard will do more for you than a celebrated high-polyphenol oil bought two years ago and left by the hob.
Where a number is offered, ask for the method and the date. Where no number is offered, ask about grade, harvest and extraction. And treat the diet research for what it is: strong evidence for a pattern of eating in which good olive oil plays a central part, rather than a warrant for any bottle in particular.
